DebitModRelief is a workers’ compensation program operated by Excelsure Insurance Services, an independent insurance agency (CA License #0E89987), located at 18377 Beach Boulevard, Suite 325, Huntington Beach, CA 92648. This policy explains what we collect through this website and what we do with it.

When you submit a review request, we collect:

• Contact details — your name, business name, phone number, and email address.
• Business details — industry, state, employee count, annual payroll, current workers’ compensation premium, experience modification rate, policy renewal date, claim and non-renewal history, and any notes you add.
• Technical and referral data — IP address, browser user agent, the page the form was submitted from, and any campaign parameters (such as UTM tags) present in the link you arrived through.

We do not ask for and do not want Social Security numbers, bank or card details, or individual employees’ medical information through this website. Please do not submit them here.

• To respond to your request and prepare a coverage review.
• To approach insurance carriers, wholesalers, and program markets on your behalf — only for markets you have agreed we may approach.
• To contact you by phone, text message, or email about your request and, where you have not opted out, about related insurance services.
• To maintain our records and meet insurance regulatory, licensing, and recordkeeping obligations.

By submitting the form and ticking the consent box, you agree that Excelsure Insurance Services may contact you at the phone number and email address you provided, including by autodialed or prerecorded call and by SMS, about your workers’ compensation request. Consent is not a condition of purchasing any product or service. Message and data rates may apply, and message frequency varies. Reply STOP to opt out of text messages or HELP for help. We share the information you submit with:

• Insurance carriers, wholesale brokers, and program administrators we approach on your behalf.
• Service providers that operate our systems — for example, our website host, email provider, and customer relationship management platform — under contracts limiting their use of the data to providing those services.
• Regulators, auditors, and legal counsel where required by law or to establish or defend legal claims.

We do not sell your information, and we do not share it with unaffiliated agencies or lead aggregators.
